In order to participate in NY Coed Soccer LLC's (The Released) soccer leagues, I understand, acknowledge, and agree with the following:
Participating in any athletic event is an extreme test of a person's physical and mental limits and carries with it the potential for death, serious injury and property loss. The risks include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of athletes, equipment, vehicular traffic, actions of other people including but not limited to, participants, volunteers, spectators, referees, league organizers, and lack of hydration.
I agree to comply with all rules and regulations for participation in NY Coed Soccer programs. I agree to inspect the equipment and premises to be used prior to participation and if I believe that anything is unsafe, I will inform league officials, team captains, referees of the issue and refuse to participate if not corrected.
I hereby assume all of the risks of participating in NY Coed Soccer's programs. I realize that liability may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained or controlled by them or because of their possible liability without fault.
I certify that I am physically fit, have sufficiently trained for participation in the event and have not been advised otherwise by a qualified medical person that I cannot participate. I acknowledge that this Accident Waiver and Release of Liability form will be used by NY Coed Soccer in events in which I may participate and that it will govern my actions and responsibilities at said events.
In consideration of my application and permitting me to participate in this event, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:
Waive, release and discharge from any and all liability for my death, disability, personal injury, property damage, property theft or actions of any kind which may hereafter accrue to me or my traveling to and from this event, the organizer and the following entities or persons: Their directors, officers, employees, volunteers, representatives, and agents, the event holders, event sponsors, event directors, event volunteers, and event officials.
Indemnify and hold harmless the entities or persons mentioned in this paragraph from any and all liabilities or claims made by other individuals or entities as a result of my or any actions during this event.
I am voluntarily participating in a NY Coed Soccer program. I hereby consent to receive medical treatment, which may be deemed advisable in the event of injury, accident and/or illness during the league. The Accident Waiver and Release of Liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
While we are playing during a global pandemic we understand that a player’s health extends past what typically occurs on a soccer field.
NY Coed Soccer will endeavor to ensure that proper health measures are taken, voluntary disclosure forms as well as ensuring our players have masks while around other NY Coed Soccer staff and players. We ask that you ensure you have not been exposed to COVID-19 either in being in a state or area that New York State recommends quarantining before entering the state or around a person diagnosed with COVID-19 before participating in NY Coed Soccer activities.
However, there is no perfect system to ensure the virus does not spread. Thus, if you choose to participate in any NY Coed Soccer activity you do so waiving any liability or responsibility to NY Coed Soccer or any of its staff. All forms of waiver that exist in this agreement shall extend to any COVID-19 related ailments.
I hereby certify that I have read this document and I understand its contents.
By agreeing, I acknowledge that I have read this waiver and release of liability and I fully understand that I have given up substantial rights, and I submit this form having given up substantial rights, and I submit it knowingly and voluntarily without any inducement or duress.
